Job Description: Licensed Practical Nurse (LPN)

Location: Raleigh, NC

Department: Clinical Services – Outpatient Mental Health & Substance Use

Reports To: Registered Nurse / Clinical Director / Owner Daniel Jack

FLSA Status: Non-Exempt

Employment Type: Full-Time

Schedule: Monday – Friday, 9:00 AM – 5:00 PM. No nights, weekends, or on-call.

Position Purpose

The Licensed Practical Nurse provides direct, compassionate nursing support under the supervision of a Registered Nurse for patients receiving outpatient mental health and substance use services. You help patients feel safe, remain medically stable, and follow their treatment plans with dignity. Essential Duties & Responsibilities

  1. Patient Care: Obtain vital signs, weights, and required screenings during admission and follow-up visits. Observe and promptly report changes in a patient’s physical or mental condition to the supervising RN or provider.
  2. Medication Administration: Administer oral, intramuscular (IM), and subcutaneous medications in accordance with provider orders and North Carolina Board of Nursing (NCBON) regulations. Complete Medication Administration Record (MAR) documentation accurately and on time.
  3. Assessments & Documentation: Assist the Registered Nurse with nursing assessments, vital sign monitoring, and observation of psychotropic medication side effects. Document all patient care activities in the electronic health record (EHR) in compliance with 10A NCAC 27G.0205 standards.
  4. Patient Education: Reinforce education provided by the RN or healthcare provider regarding medication adherence, possible side effects, wellness practices, and chronic disease management using supportive and easy-to-understand language.
  5. Treatment Team Support: Participate in weekly Person-Centered Plan meetings and contribute clinical observations that assist the interdisciplinary team in updating patient goals and treatment interventions.
  6. Safety & Crisis Support: Assist with patient de-escalation, safety monitoring, and crisis response during clinic hours. Immediately notify the supervising RN or provider of any medical or behavioral emergencies.
  7. Compliance: Follow infection prevention protocols, medication storage and disposal procedures, CLIA-waived testing requirements, and HIPAA regulations. Help maintain a clean, organized, and safe clinical environment.
  8. Care Coordination: Schedule laboratory appointments, communicate with pharmacies, and assist with coordinating follow-up care with primary care providers and community resources under RN supervision.

Note per NCBON: The Licensed Practical Nurse practices under the supervision of a Registered Nurse, physician, or other authorized healthcare provider. Independent nursing assessments, care plan development, and delegation are outside the LPN scope of practice.

Minimum Qualifications

  1. Education & License: Graduate of an accredited Licensed Practical Nurse program with an active, unrestricted North Carolina LPN license in good standing.
  2. Experience: One or more years of LPN experience is preferred. Experience in psychiatric, behavioral health, substance use treatment, long-term care, or outpatient clinical settings is highly desirable. New graduates with strong clinical experience are encouraged to apply.
  3. Certifications: Current BLS/CPR certification required. CPI or de-escalation training must be completed within 90 days of employment.
  4. Skills: Strong medication administration skills, accurate vital sign monitoring, EHR documentation, effective patient communication, teamwork, and a commitment to compassionate, respectful patient care.

Physical & Work Environment

This position is based in our outpatient clinic in Raleigh, North Carolina. Candidates must be able to sit or stand for extended periods, lift up to 25 pounds, respond quickly during emergencies, and occasionally participate in community outings with staff.

Compensation & Benefits

Benefits: Medical, dental, and vision insurance after 60 days of employment, 12 days of paid time off (PTO), 7 paid holidays, CEU reimbursement for license renewal, malpractice insurance coverage, and a consistent Monday through Friday schedule from 9:00 AM to 5:00 PM with no weekends or on-call responsibilities.
